The execution of convicted murderer Christa Pike on Sept. 30 at the Riverbend Maximum Security Institution in Nashville resulted in Pike becoming unconscious, requiring intubation and connection to a mechanical ventilator, according to a court filing by her attorneys on Oct. 2.
The incident marks Tennessee’s second failed execution attempt this year. In response, Governor Bill Lee ordered a halt to all death‑penalty executions for the remainder of 2026 and directed an independent review of the Riverbend execution protocol. The governor’s office indicated that an interim commissioner will be appointed to lead the Department of Correction while the review proceeds.
In a statement, Strada said he believes his agency’s personnel followed proper procedures during the execution, but that stepping aside “is in the best interest of Tennesseans as the independent review moves forward.” He added that he will cooperate fully with the investigation.
Governor Lee praised Strada’s service, noting that the commissioner “has served with integrity” and expressing appreciation for his willingness to put the interests of Tennesseans first during the difficult moment. Lee’s statement also underscored the administration’s commitment to a thorough, independent assessment of the execution process.
Pike’s legal team has accused Department of Correction officials of mishandling the execution, alleging that procedural errors contributed to the inmate’s severe medical complications. The pending review is expected to examine those allegations and determine whether changes to the state’s execution protocols are required.
